MRI MAMMOGRAM BILATERAL BREAST


Protocol- Multiplanner T1, T2. & STIR sequences taken.

Right breast shows-
 E/O T2/STIR hyperintensity of size 3.1×2.2×1.8 cm with illdefined irregular margined soft tissue lesion in lower and outer quadrant of right breast extending & infilterating the skin however no e/o scarring seen likely neoplastic (BIRADS-5).
Reduced breast size.
 E/O T2/STIR hyperintensityTwo lesions of size 8x6mm & 8x2mm likely cysts (BIRADS-2) 
Rest of breast parenchyma shows normal fibroglandular tissue.
Nipples shadow appears normal.
Axillary lymphadenopathy (12x7mm)

Left breast shows-
Tiny cyst of 2x3mm size at 12 o çlock position of left breast likely benign lesion in upper quadrant (BIRADS-2)
Rest of breast parenchyma shows normal fibroglandular tissue.
Nipples shadow appears normal.
Axillary lymphadenopathy (12X6)
 
IMPRESION:-
Multiple tiny cysts seen in bilateral breast (BIRADS-2)  
E/O T2/STIR hyperintensity of size 3.1×2.2×1.8 cm with illdefined irregular margined soft tissue lesion in lower and outer quadrant of right breast infilterating the ajacent skin likely neoplastic (BIRADS-5) with multiple axillary lymphadenopathy.

ADVICE-  Histopathological co-relation


NOTE:
There are four descriptors for breast density on mammography in the 5th edition of BI-RADS 
a: the breasts are almost entirely fatty
b: there are scattered areas of fibroglandular density
c: the breasts are heterogeneously dense, which may obscure small masses
d: the breasts are extremely dense, which lowers the sensitivity of mammography
 
Breast imaging studies are assigned one of seven assessment categories:

BI-RADS 0: incomplete
need additional imaging evaluation (additional mammographic views or ultrasound) and/or
for mammography, obtaining previous images not available at the time of reading

BI-RADS 1: negative
symmetrical and no masses, architectural distortion, or suspicious calcifications

BI-RADS 2: benign
0% probability of malignancy

BI-RADS 3: probably benign
<2% probability of malignancy
short interval follow-up suggested

BI-RADS 4: suspicious for malignancy
2-94% probability of malignancy
for mammography and ultrasound, these can be further divided:
BI-RADS 4A: low suspicion for malignancy (2-9%)
BI-RADS 4B: moderate suspicion for malignancy (10-49%)
BI-RADS 4C: high suspicion for malignancy (50-94%)
biopsy should be considered

BI-RADS 5: highly suggestive of malignancy
>95% probability of malignancy
appropriate action should be taken

BI-RADS 6: known biopsy-proven malignancy.
